Participants in the AMPED program received medication management, lipid management and guidance on healthy eating, smoking cessation, and controlling hypertension and diabetes. They also received patient education and exercise counseling. For patients who received AMPED services and demonstrated appropriate adherence, hospitalizations for CAD decreased by 23 percent and the cost of hospitalization decreased by 33 percent.
